New Patients

Please complete the new patient form by clicking on the PDF form and bring with you to your scheduled appointment.

Items to bring with to your appointment:

  • Completed new patient paperwork
  • Insurance cards
  • Photo ID
  • All medications and supplements or a list of current medications and supplements with the dosage
  • We will have you sign a consent yearly and a payment options form at each visit

Who We Serve

Nurse Practitioners have been providing primary, acute and specialty healthcare to patients of all ages and stages of life for over 50 years. Nurse Practitioners assess patients, order diagnostic tests, make diagnoses, and manage treatment plans including prescribing medications.

Payment Options

We all know that health care today is just not the same. People often take insurance coverage with high deductibles.  Here at Heckert Health Center, LLC, we are providing you with an option.  At each visit you choose a Payment Path:

Path #1 - Direct Pay: Services are offered at a direct pay (cash, credit card, check) rate.  Payment is expected in full for all services provided the day of the visit. 

Path #2 - Use Insurance: A copy of your current insurance card and full payment of your co-pay is required before your visit. Please be sure to check with your insurance provider before your visit.

Failure to keep a scheduled appointment and give a 24-hour notice will generate a $30.00 No Call No Show Courtesy Fee.

Our hours are from 8:00 a.m. - 4:30 p.m., Monday to Friday. To schedule an appointment during office hours, please call us at (402) 371-0263. If you are unable to keep your appointment, please let us know as soon as possible.
